MANDATORY PROTECTIVE EQUIPMENT NO EXCEPTIONS!!!

HEAD GEAR, FOOT PADS(dipped foam only), HAND PADS (dipped foam only), GROIN/ATHLETIC PROTECTOR & A MOUTHPIECE (chest protectors are optional).

ALL SPARRING GEAR MUST BE IN GOOD REPAIR. NO TAPE OR OVERLY WORN EQUIPMENT WILL BE ALLOWED.
ALL COMPETITORS MUST WEAR ACCEPTABLE MARTIAL ARTS UNIFORM, NO TANK TOPS OR T-SHIRTS.

LIABILITY WAIVER

I hereby submit this registration and liability waiver form to participate in the San Diego Friendship Tournament. I certify that the
above information is true and correct and hereby release, discharge and waive any and all responsibility of the United World
Taekwondo Association, Robinson’s Taekwondo Centers, Folsom Taekwondo Center, Pourarian’s Taekwondo, Pacific Athletic
Club, the Tournament Organizing Committee, tournament directors, referees, instructors, coaches, medical staff and other
competitors from liability for any injury, including death, and for damage to or loss of property which may be suffered by myself
arising out or, or in any way resulting from or attributing in whole or in part to my traveling to, training for, being coached in, using
any sports equipment, or participating in the San Diego Friendship Tournament.
